Job
Objectives:
Provide
goal-directed, medically necessary services and to support adults who have a
diagnosed mental illness which causes functional impairment and contributes to
repeated criminal justice system contact.
Essential
Job Functions:
Direct
Service Provision
Provide
Targeted Case Management (TCM) for each assigned client to aid client in
achieving goals identified in the individualized treatment plan. TCM
interventions may include: treatment planning, collateral contacts, access to
supports and service coordination.
Essential
job tasks include but are not limited to the following:
Provide
assistance to clients in making collateral contacts in addition to completing
application for food stamps, medical services, energy bills, entitlements,
housing, vocational services and legal obligations.
Provide
phone support to clients.
Confer
with client caregivers, families, physicians, lawyers, community service
providers and other professionals to secure and coordinate services.
Provide
Community Psychiatric Supportive Treatment (CPST) for each assigned client as
needed to assist client to achieve treatment goals. CPST interventions may
include the following: assistance in improving symptom self-management;
individual therapeutic intervention, strengths assessment and planning;
assistance to assure consumer income maintenance; medication education; crisis
management; identifying and utilizing social support resources to help consumer
maintain independent living in the community; support consumer in attending
court hearings, reporting to probation and following probation requirements as
needed; or assistance to achieve academic progress
Essential
job tasks include but are not limited to the following:
Accompany
individual clients to appointments and other community locations as needed;
assist in developing housing and transportation options.
Assist
client in tracking and completing court requirements as needed.
Monitor
gross psychological and physical functioning of client.
Assist
clients in acquiring needed documentation such as birth certificates, social
security, etc.
Assist
with treatment admissions and discharge (includes involuntary
hospitalizations). When indicated work with State Hospital Liaison and assigned
clients to implement discharge and reintegration to the community. When
indicated work with the Diversion Specialist and Inpatient Coordinator to plan
and follow-up with consumers in local hospital.
Provides
a minimum of 1040 hours of client service activity, both billable and
non-billable, to assigned clients annually.
Provides
initial service to all assigned clients within 5 days of the date of their
intake for new clients and within 5 working days for clients internally
transferred from another case manager.
Treatment
Planning
Review
intake assessment and initial treatment plan completed by SCOAP QMHP for all
assigned clients.
Meet
with client to complete strengths assessment, assess resources and desires
related to specific life domain areas including independent living, vocational/
educational, social support, health and discuss treatment goals and available
services.
Complete
second treatment plan review with client and treatment team members no later
than 60 days from the date of the initial intake and no earlier than 14 days
prior to the due date.
Conduct
treatment plan review and update treatment plan documenting progress toward
established treatment goals no later than every 90 days following the date of
intake or as needed to reflect client's progress and treatment needs.
Ensures
client participation in treatment planning process.
Coordinate
with other service providers to review client's progress and update treatment
plan goals, objectives and interventions.
Documentation
Completes
all progress notes within 48 hours of service delivery with/or on behalf of the
client.
Documentation
meets minimum Medicaid standards for content and quality.
Completes
Notes for Record to ensure the completeness of the medical record.
Enters
clinical transactions in PsychConsult for no shows and cancellations.
Attend
and participate in all Compliance and Quality Management trainings. Forwards
progress notes to COMCARE Compliance Officer for first 60 days of employment
and to supervisor thereafter as directed.
General
Administrative Duties
Identifies
sources of information needed to provide comprehensive mental health treatment
and secures the necessary COMCARE releases of information. Monitors releases of
information for annual update.
Submits
necessary referral forms to team leader or QMHP for approval or assignment in
PsychConsult.
Ensures
client's financial information is current and up-to-date in the medical record.
Maintains
updated diagnosis in client’s medical record.
Review
and respond to UR Staff Feedback Forms.
Inform
office specialist of individual client court dates and probation dates.
Complete
court reports and probation progress reports for clients as required.
Investigate
community support resources and referral procedures to meet client’s needs.
Perform
other duties as required.
Employee
Development
Complete
required Case Management On-line Training at first available opportunity and
within first 60 days of employment.
Complete
required Basic Case Management Training at the first available opportunity and
within first 6 months of employment.
Participates
in continuing education to stay abreast of adult mental health issues and to
develop enhanced skill set to meet the needs of clients.
Participate
in regularly scheduled supervision and shift meetings with immediate supervisor
as directed.
Participate
in team meetings to facilitate treatment options for clients.
Job
Standards:
Bachelor's
degree or be equivalently qualified by work experience or a combination of work
experience in the human services field and education, with one year of
experience substituting for one year of education. Possess demonstrated
interpersonal skills, ability to work with persons with severe and persistent
mental illness and/or severe emotional disturbance, and the ability to react effectively
in a wide variety of human service situations. Must complete basic case manager
training; intermediate case manager training within first 6 months of
employment.
Must
have a valid Kansas Driver s license, current proof of automobile insurance and
be able to safely operate a motor vehicle.
Ability
to cope with psychotic episodes of adults.
Knowledge
of community services and benefit programs.
Ability
to develop and maintain rapport with patients, the general public and all
levels of staff.
Ability
to keep client documentation current.
Possess
the awareness and judgment to intervene appropriately when dealing with
escalating emotional situations
Possess
the ability to negotiate several flights of stairs at a time and move
effectively in varied settings throughout the community
Possess
the mobility and judgment to execute personal protective measures
Able
to calmly and effectively deal with agitated individuals or high risk

Environment:
Office
based employment with work in various community settings including, Sedgwick
County Adult Detention Facility, private homes, community centers, and other
collateral professional settings.

Must
pass KBI background check and pre-employment urine drug screen.
Valid Kansas Driver’s License
Required. Must meet Driver Qualification
standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage
policy.
