court personnel, and other resources to assess the current functioning of
clients while detained and to anticipate their discharge to the community.
Outreach clients while detained at SCADF Mental Health
Pod.
Coordination with client s community based
treatment teams to update them and develop comprehensive plans for client s
return to the community.
Scheduling appointments on
client s behalf to support establishment or maintenance of medication and
therapeutic support and treatment.
Coordination with the client to
encourage ownership in their recovery, engage them in their treatment planning
and goal setting, and develop a crisis plan.
5% Treatment
Planning
Review intake assessment completed by QMHP
for all assigned clients.
Meet with client to complete DLA
20, discuss treatment goals and available services.
Complete initial treatment plan
and crisis alert plan with client and treatment team members no later than 30
days from the date of the intake.
Conduct treatment plan review
and update treatment plan documenting progress toward established treatment
goals every 90 days following the date of intake or as needed to reflect client
s progress and treatment needs.
Coordinate with other service
providers to review client s progress and update treatment plan goals,
objectives and interventions.
10% Documentation
Complete all progress notes
within 48 hours of service delivery with/or on behalf of the client.
Complete Notes for Record and
call logs to ensure the completeness of the medical record.
Enter clinical transactions in Psych Consult
for no shows and cancellations.
Forward progress notes to
supervisor for approval for first 60 days of employment or as directed.
15% General
Administrative Duties
Identify sources of information
needed to provide comprehensive mental health treatment and secure the
necessary COMCARE consents to release information. Monitor releases of
information for annual update.
Submit necessary referral forms
to team supervisor for approval or assignment in Psych Consult.
Ensure client’s financial
information is current and up-to-date in medical record.
Enter AIMS data for all clients,
including closures, in a timely manner.
Maintain updated diagnosis in client s
medical record.
Review and respond to UR Staff Feedback
Forms.
Complete and submit required
court and probation progress reports to supervisor for review prior to
probation meeting or court hearing.
Investigate community support
resources and referral procedures to meet client s needs.
Perform other duties as required.
10% Documentation
Content Quality Review
Complete documentation according
to minimum Medicaid and agency standards for content and quality.
Job Standards:
Bachelor s degree or be equivalently qualified by work
experience or a combination of work experience in the human services field and
education, with one year of experience substituting for one year of education.
Possess demonstrated interpersonal skills, ability to
work with persons with severe and persistent mental illness and/or severe
emotional disturbance, and the ability to react effectively in a wide variety
of human service situations.
Meet the specifications as outlined in the CMHC licensing
standards.
Security clearance at Sedgwick County Adult Local
Detention Facility is required.
Must complete basic case manager training; intermediate
case manager training within first 6 months of employment, may require
overnight stay.
Must have a valid Kansas Driver s license, current proof
of automobile insurance and be able to safely operate a motor vehicle.
Additional qualifications include: Possess the awareness
and judgment to intervene appropriately when dealing with escalating emotional
situations
Possess the ability to negotiate several flights of
stairs at a time and move effectively in varied settings throughout the
community.
Possess the mobility and judgment to execute personal
protective measures.
Ability to calmly and effectively deal with agitated
individuals or high risk situations.
Demonstrate autonomous critical thinking skills.
Demonstrate good professional judgment and
decision-making skills.
Self-motivation and the ability to work with minimal
direct supervision.
Demonstrate good time management & organizational
skills in a fast-pace environment.
Ability to handle competing urgent priorities.
Possess basic or intermediate computer skills: able to
learn complex software related to an electronic medical record. This position requires driving.
