Job Objectives:

Exciting position in new program focused on improving overall health of persons with severe mental illness and/or chronic phycial health conditions.  Positions is responsible for coordinating the health care needs of health home participants as defined in the Health Action Plan via core health home activities which include:  care coordination, health promotion, comprehensive transitional care, member and family support and referral to community supports and services.  This is done in a team based approach to care and is intended to optimize the overal health of those served.

 

Essential Job Functions:

25%    Health Home Service Provision

Provide health home services to individuals who have a severe mental illness and those whom have or are at risk of having chronic health conditions such as asthma, diabetes, hypertension, weight management issues, etc.

Engage members and families as partners in their care and develop positive, hopeful and trusting relationships.

Address barriers for success:  i.e.: low income, housing, transportation, health condition, etc.

Key role in overall management of the member’s health action plan. Is also responsible for routine and periodic reassessment and revision of the health action plan (HAP).

Assist members during medical appointments, as warranted. in understanding the relevant details of the visit to increase health literacy and follow through with recommendations.

Utilize behavioral change strategies to help members and family/supports implement changes (i.e.:  maintaining a food diary when trying to manage or lose weight, identifying coping strategies to manage complex tasks, etc.

25%    Care Coordination

Outreach high risk members to encourage active participation in health home activities.

Facilitate the transition from inpatient or outpatient medical care to home and other health service systems and coordinate behavioral health care transitions with liaisons and treatment team members.

Coordinate access to preventive and health promotion services for chronic conditions as well mental illness and substance use disorders.

Make collateral contacts with doctors, family members, designated team members, etc. to assure the needs of the member are being met.

Be the system navigator and point of contact for members.

Coordinate and provide access to individual and family supports.

Coordinates appointments as needed.

20%    Teaching & Education

Engage members to better understand health conditions/literacy and motivate them to self-manage and partner in their overall health care.

Provide individual and group health promotion/education and coaching to members and their supports, including but not limited to the benefits of a healthy lifestyle, good nutrition, physical activity and chronic disease management.

Encourage and emphasize self-direction and skill development that promotes healthier living.

Engage family/supports through education and health promotion activities that will help members achieve their goals.

          Utilize motivational interviewing techniques to facilitate change.

          Use a step-by-step process when teaching skills

          Utilize positive reinforcement and encouragement

15%    Record Keeping

Complete documentation in an accurate and timely manner and in compliance with agency, state and federal requirements for coding and content.

          Ensure confidential communication of patient health information.

15%    Other Administrative Duties

Monitor and follow up to ensure that needed care is not only offered, but accessed.

          Demonstrate value of a team approach to care. 

          Support and grow the member’s confidence and commitment to change.

Identify and foster strong working relationships with other community providers.

Understand and work toward achieving program outcomes as defined by contract and as they relate to overall population health.

Job Standards:

Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience and training in the field of behavioral or physical health care. 

Possess demonstrated interpersonal and written skills and ability to thrive in a team based approach to care.

A personal focus on living a healthy lifestyle or willingness to model a healthy lifestyle with health home members. 

          Ability to work with a culturally diverse population.

          Knowledge of community services and benefit programs.

Ability to develop and maintain rapport with patients, the general public and all levels of staff.

Possess basic or intermediate computer skills: able to learn complex software related to an electronic medical record.

Valid Kansas Driver’s License and proof of automobile insurance required.  Must be able to safely operate a motor vehicle, and meet Driver Qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage. This position does require driving.

Work Environment: 434 N Oliver Wichita, KS

Primarily office work with some time spent in various community locations

Usual working days/hours:  Monday - Friday 8am - 5pm

Must pass KBI background check and pre-employment urine drug screen.

Valid Kansas Driver’s License Required.  Must meet Driver Qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.This position may be filled or closed to further application without notice.
