Job Objectives:
Seeking an outgoing and self-motivated individual to work
in this exciting and innovative new team based care program. The Care Manager will serve as a skilled
health behavior change specialist and will manage a broad array of physical
health needs in addition to behavioral healthcare needs with a strong focus on
managing chronic health conditions. The
health home care manager will provide timely, comprehensive, high quality health
coordination based on a whole person approach to care. The whole person approach to care addresses
clinical and nonclinical care needs through a Health Action Plan (HAP) with six
core Health Home services: Comprehensive
Care Management, Care Coordination, Health Promotion, Comprehensive
Transitional Care, Member and Family Support and Referral to Community Supports
and Services. This position will work
closely in coordination with all health providers serving the health home
participant, including providing support, skill building and clinical guidance
to care coordinators on the team.
Essential Job Functions:
20% Identification
of health needs
Completes health risk
assessments on assigned health home participants.
Assists in the development of a
Health Action Plan (HAP) that is based on the physical,
behavioral health, and
social needs of the health home participant.
Tracks that all required
assessments and screenings are completed for each person on
their caseload in
accordance with expected quality measures for Kansas Medicaid
Health Homes, and
follows up as necessary.
Identifies population health
improvement activities to optimize health for those in
COMCARE’s health home.
20% Health
Promotion and Wellness
Champions healthy lifestyle
changes, including tobacco preventions and cessation,
diabetes self-management,
nutrition and physical activity.
Assists in developing and
enhancing wellness initiatives such as but not limited to wellness support
groups, periodic employee and health home member education classes, health home
member health fairs and screenings.
Support and educate care
coordination staff on health coaching, chronic diseases, healthy lifestyle
choices and medications.
Identify and develop health
promotion materials and develop resource library for health home members.
20% Patient
Engagement Activities
Champions
preventive care efforts by serving as skilled health behavior change
specialist.
Makes contact within 72 hours of
hospital discharge and conducts medication
reconciliation with input from the
member’s primary care physician if necessary.
Coordinates care with external
healthcare providers, including PCP, pharmacies,
MCO’s as warranted, FQHC
clinical staff or other specialty providers.
Initiate member education based
on a health coach model on identified needs of the member and/or those
providing care.
Inspire
readiness for behavior change.
Improve
health literacy for members

Coordination
Identifies alerts and
coordinates with the care team on disease management
indicators, drug alerts,
etc.
Assists in identification of
supports for members, family, support persons needed
to manage the member’s
condition and assist them to access these supports.
Identify and help to remove care
gaps.
Assist in simplifying the care process
for members – seamless transitions when possible.
Identify
and foster strong working relationships with other community providers.

Partner
with the members and explore how they desire to achieve better help.

Positions Supervised:
No direct reports, but will provide behavioral health and chronic
condition support, guidance and training to health home care coordinators and
other COMCARE employees
Job Standards:
Must meet either of the following: Must possess a Master's Degree in a Human Service field
and hold BSRB licensure in Kansas or be
licensed as a registered nurse with the Kansas Board of nursing.
A minimum of 1 -3 years related work experience in
behavioral or physical health care.
Preference given to applicants with relevant physical healthcare
experience related to treatment of chronic conditions and/or strength in
motivational interviewing.
Possess demonstrated interpersonal and written skills and
ability to thrive in a team based approach to care.
Valid Kansas Driver’s License, proof of automobile
insurance required, and be able to safely operate a motor vehicle. Must meet Driver Qualification standards as
set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.
Strong organizational skills are essential.
Must have strong documentation skills and ability to meet
the documentation requirements as defined in the State Medicaid Health Home
Manual.
Understanding of population health management principles.
A personal focus on living a healthy lifestyle or
willingness to model a healthy lifestyle with health home members.

Monday - Friday 8am - 5pm
Work Environment: 635 N Main Wichita, KS
Primarily office work with some time spent in various
community locations. This position requires driving.

meet Driver Qualification standards as set forth in Sedgwick County’s Fleet
Vehicle Operation and Usage policy.
