The Healthy Babies CL provides direct in-home services to pregnant and newly parenting women in Sedgwick County through assessment, education and support. The program’s focus is on reduction of infant mortality, low birth weight and prematurity among high risk moms. This is an exempt position, per Sedgwick County guidelines, and the home visitor’s workweek may occasionally exceed 40 hours.
Essential Job Functions:
• Home Visits • Documentation • Maternal Child Health Education • Collaborates with the HB program providers and participates in joint home visits per client need • Makes referrals to other community agencies based on individual client issues • Outreach • Submits client charts for closure within the timeframes outlined by program policy. • Checks physical mailbox and email on a daily basis. Responds to emails and Outlook notices no later than 48 business hours after receipt • Actively participates in scheduled Reflective meetings with Supervisor • Attends all required trainings and meetings • Attends and participates in monthly Community Forums • Will respond to a public health emergency or disaster situation as directed by supervisor • Employees who are required to wear respirators in emergency situations for emergency response will be enrolled in the respiratory protection program that includes annual fit testing and medical evaluation • Complete ICS 100 and 700 (Incident Command System) training within 30 days of employment with results provided to designated Public Health Emergency Management staff • Proof of vaccination against certain diseases within 10 days of employment • Participation in the TB screening program that includes baseline screening for M. Tuberculosis infection within 10 days of employment • Participation in the Blood borne Pathogen Exposure Control Plan that includes training, proof of Hepatitis B immunity or vaccine, or vaccine taken within 10 days of employment
Bachelor’s degree in Social Work with BSRB licensure or Bachelor's degree in Early Childhood plus a minimum of one year field case management, home visiting experience with multi-stressed families, or any equivalent combination of training and experience, which provides the required knowledge, skills, abilities and competencies. Bilingual preferred. Offers of employment will be made contingent upon passing a drug screen, and upon satisfactory evaluation of background and reference checks. This position requires daily use of a personal vehicle and a valid State of Kansas Driver’s License and the applicant must meet Driver Qualification standards as set forth in Sedgwick County s Fleet Vehicle Operation and Usage policy (R#225-1999). Applicants with a history of 2 or more moving violations in 24 months or 3 or more in 36 months will not be eligible for employment with Sedgwick County in this position. • Current CPR certification • For those with a bachelor’s degree in social, must have current Kansas license through KSBSRB • Knowledge of community resources to address client barriers. • Knowledge on Family Dynamics and Parent-Child Issues • Knowledge of HIPAA regulations. • Skill in the use of a personal computer • Interviewing skills • Bilingual English/Spanish skills a plus • Ability to utilize program curriculum as appropriate • Ability to demonstrate cultural sensitivity in working with high acuity, multi-stressed families of diverse socio-economic backgrounds • Ability to work as a member of a team • Ability to climb stairs, navigate uneven and rough terrain and lift/carry up to 40 lbs. • Ability to provide home visits in a variety of settings, including home, community, school, and office settings • Ability to maneuver self and equipment within environmental conditions and structures to accomplish job functions • Ability to travel within Sedgwick County in all weather conditions • Responsible for ensuring items given to clients meet program guidelines governing boundaries and enabling issues, as well as meeting county and grant guidelines. • This position works collaboratively with other areas of the Health Department, as well as partner agencies, to ensure appropriate and complete wraparound services for our clients. Represents Healthy Babies and does presentations at health fairs, community events, middle and high schools and for other community agencies as requested. • Possess a valid Kansas driver’s license with current insurance & registration • All Health Department positions require valid Driver’s License and proof of Liability Insurance.
