Provide goal-directed, medically necessary services and support to adults who have mental illness and substance abuse problems contributing to repeated criminal justice system contact.
Essential Job Functions:
Direct Service Provision • Provide Community Psychiatric Supportive Treatment (CPST) for each assigned client no less than one time per week, or as indicated on treatment plan, to assist client to achieve treatment goals. CPST interventions include: assistance in improving symptom self-management, strengths assessment and planning, assistance to assure consumer income maintenance, individual therapeutic intervention, medication education, crisis management, assistance in understanding and completing court requirements, or assistance to achieve academic progress. • Provide Attendant Care (AC) for assigned clients to assist client in achieving goals identified in the individualized treatment plan. AC interventions include: direct support, supervision and/or cueing. • Provide non-billable client service activity. Non-billable service activity includes providing outreach services, screening potential program participants, responding to the immediate needs of walk-ins, and arranging for releases from the jail to the program. Treatment Planning • Review intake assessment completed by QMHP for all assigned clients. • Meet with client to complete strengths assessment, discuss treatment goals and available services. • Complete initial treatment plan and crisis alert plan with client and treatment team members no later than 30 days from the date of the intake. • Conduct treatment plan review and update treatment plan documenting progress toward established treatment goals every 90 days following the date of intake or as needed to reflect client s progress and treatment needs. • Coordinate with other service providers to review client s progress and update treatment plan goals, objectives and interventions. Documentation • Complete all progress notes within 48 hours of service delivery with/or on behalf of the client Complete Notes for Record to ensure the completeness of the medical record. • Enter clinical transactions in PsychConsult for no shows and cancellations. • Forward progress notes to supervisor for approval for first 60 days of employment or as directed. General Administrative Duties • Identify sources of information needed to provide comprehensive mental health treatment and secure the necessary COMCARE consents to release information. Monitor releases of information for annual update. • Submit necessary referral forms to team supervisor for approval or assignment in Psych Consult Ensure client s financial information is current and up-to-date in medical record. • Enter AIMS data for all clients, including closures, in a timely manner. • Maintain updated diagnosis in client s medical record. • Review and respond to UR Staff Feedback Forms. • Complete and submit required court and probation progress reports to supervisor for review prior to probation meeting or court hearing. • Investigate community support resources and referral procedures to meet client s needs. • Perform other duties as required. Documentation Content Quality Review • Complete documentation according to minimum Medicaid and agency standards for content and quality.
• Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. • Previous Case Management experience required. • Possess demonstrated interpersonal skills, ability to work with persons with severe and persistent mental illness and/or severe emotional disturbance, and the ability to react effectively in a wide variety of human service situations. • Meet the specifications as outlined in the CMHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry and motor vehicle screens. • Security clearance at Sedgwick County Adult Local Detention Facility is required. • Must complete basic case manager training; intermediate case manager training within first 6 months of employment, may require overnight stay. • Must have a valid Kansas driver’s license, current proof of automobile insurance and be able to safely operate a motor vehicle. • Must maintain valid Kansas driver’s license and proof of automobile insurance at all times. • Additional qualifications include: § Possess the awareness and judgment to intervene appropriately when dealing with escalating emotional situations § Possess the ability to negotiate several flights of stairs at a time and move effectively in varied settings throughout the community § Possess the mobility and judgment to execute personal protective measures § Able to calmly and effectively deal with agitated individuals or high risk situations § Demonstrate autonomous critical thinking skills § Demonstrate good professional judgment and decision-making skills § Self-motivated § Able to work with minimal direct supervision § Demonstrate good time management and organizational skills in a fast-paced environment § Able to handle competing urgent priorities § Possess basic or intermediate computer skills: able to learn complex software related to an electronic medical record
Background checks: KBI, DCF child abuse check, adult abuse registry, Sedgwick County Adult Detention Facility security clearance and motor vehicle screening in addition to a pre-employment drug screen.
