Provide goal-directed, medically necessary services and support to clients, between the age of 16-22, with severe emotional disturbances and their families; working toward transitioning youth into independent living or adult mental health services within the community.
Essential Job Functions:
• Provide direct services to identified clients and their families by developing treatment plans, making referrals to community resources and direct therapeutic interventions, education and support and monitoring all mental health services to ensure integrated treatment and maximum benefit to the client. • Includes developing curriculum to address needs of transition age youth, implementation and on-going evaluation of this psychosocial rehabilitation group. On-site consultation and support to COMCARE staff working with clients in the target population. • Provide educational support services, which will mean assistance with traditional primary and secondary education, as well as non-traditional education such as GED programs. • Provide employment support service, which includes assisting the client to obtain and maintain employment. • Provide housing support services, which may include assisting the client to locate and secure affordable, safe housing in the community. • Maintain working relationships with Bridges Independent Living program at the Wichita Children’s Home, Wichita Area DCF Transitional Age Specialists, COMCARE Community Support Services, Mental Health Association - Adult Services, and other community partners. • Drive personal vehicle and/or occasionally drive a county owned vehicle for business purposes (ex. Transport clients to appointments), as required; documenting and reporting mileage according to agency procedures, so that services can be provided in a timely manner; comply with agency vehicle policy at all times.
• Bachelor's degree from an accredited college or university in Social Work or a related field or any equivalent combination of training and experience which provides the required knowledge, skills and abilities. • Three years of experience as a case manager preferred. • Valid State of KS Driver's License, and meets the requirements of Sedgwick County Fleet Vehicle Operation and Usage Policy. • Possess demonstrated interpersonal skills, ability to work with persons with severe and persistent mental illness and/or severe emotional disturbance, and the ability to react effectively in a wide variety of human service situations. • Demonstrated ability to work with transition age youth and an understanding of their developmental challenges. • Experience with adult mental health services preferred. • Possess basic or intermediate computer skills: able to learn complex software related to an electronic medical record.
