Job Objectives:

Performs general public health nursing services including physical assessments and/or provides skilled nursing services in a home environment including nursing assessments.

Essential Job Functions:

Asks questions to determine clients medical history and needs including physical, mental, social and environmental; assesses clients physical systems and implements nursing care plan.  Assesses homebound clients to determine eligibility for admission to the home health agency, other department programs and to determine health condition; assesses clients physical condition and prepares care plan according to physician consultation.  Counsels with clients, parents or caregivers and refers to physicians, dentists or other community agencies for needed health services.  Documents assessments and care given; evaluates progress and records findings according to established policies and procedures.  Communicates with client’s physician as needed.  Performs general public health nursing services in a variety of areas to include: immunizations, tuberculosis testing, family planning, sexually transmitted disease screening and treatment, preschool screenings, prenatal care, AIDS testing and counseling, cancer screening, diet counseling and other services.  Makes regular home visits and carries out skilled nursing services as directed.  May maintain billing information and statistical data for monthly reports.  May conduct home visits to pregnant women and/or parents with newborns, infants and preschoolers.  Promotes and supports breastfeeding to prenatal and postpartum women.  Promotes, conducts or participates in educational services and workshops which promote public/home health awareness, methods and procedures.  Clinically assists physicians and monitors school immunization records.  Attends monthly staff and in-service meetings and weekly care plan conferences.

Job Standards:

B.S.N. preferred.  May be a three (3) year nursing diploma graduate.  May substitute relevant past public health/home health nursing experience for degree.  Must have a current license to practice nursing in the State of Kansas.  Must be certified by the KDHE to perform physical assessments.  Requires a minimum of one (1) year of experience in public health, medical/surgical or pediatric nursing.  Must possess a valid Kansas driver’s license.

Testing/Screening Requested:

Must pass a pre-employment Physical Capacity Profile and Drug Screen.

Equipment:

Operate all job related machinery, equipment, tools and other aids as required, necessary, or needed in the performance of these job functions.

Work Environment:

Most work is performed indoors.  Indoor duties are performed in climate controlled surroundings.  May be exposed to inclement weather during outside duties.  Possible exposure to communicable diseases.  On occasion may be exposed to spills of blood or body fluids.  Must pay close mental and visual attention during all aspects of medical services.  Home environments may lack proper heat or cooling equipment.  Surroundings may be noisy.  In-home assignments may require exposure to a smoking environment.
