Job Description:

Usual working days/hours: Monday - Friday | 8:00 AM - 5:00 PM with some early evening hours

Location of work: 350 S. Broadway | Wichita, KS and in community

Job Objectives:
Provide goal-directed, medically necessary services and support to clients, between the age of 3-22, with serious emotional disturbances and their families.

Essential Job Functions:
Content of Service Activities • Provides initial service to all assigned clients within ten days of the date of their intake for new clients and within five working days for clients internally transferred from another case manager. • Identifies sources of information needed to provide comprehensive mental health treatment and secures the necessary COMCARE releases of information. Monitors releases of information for annual update. • Submits necessary referral forms to team leader or QMHP for approval or assignment in Psych Consult. • Ensures client’s financial information is current and up-to-date in the medical record. • Completes all progress notes within 48 hours of service delivery with/or on behalf of the client. • Provides a minimum of 1100 hours of client service activity to assigned clients annually. • Participates in continuing education to stay abreast of children’s mental health issues and to develop enhanced skill set to meet the needs of clients. • Drive personal vehicle and/or occasionally drive a county owned vehicle for business purposes (ex. Transport clients to appointments), as required; documenting and reporting mileage according to agency procedures, so that services can be provided in a timely manner; comply with agency vehicle policy at all times. • Performs other duties as assigned. • Provide Community Psychiatric Supportive Treatment (CPST) for each assigned client no less than one time per week, or as needed to assist client to achieve treatment goals. CPST interventions may include the following: assistance in improving symptom self-management; individual therapeutic intervention, strengths assessment and planning, assistance to assure consumer income maintenance; medication education; crisis management; or assistance to achieve academic progress. • Provide Targeted Case Management (TCM) for each assigned client to aid client in achieving goals identified in the individualized treatment plan. TCM interventions may include: treatment planning, collateral contacts, access to supports and service coordination. • Reviews intake assessment for all assigned clients. • Meet with client and family members to complete strengths assessment, discuss treatment goals and available services. • Completes first treatment plan review and crisis plan with client and treatment team members within 30 days of the intake, the next treatment plan within 60 days, and conducts subsequent treatment plan reviews and updates treatment plans documenting progress toward established treatment goals at least every 90 days or as needed to reflect client’s progress and treatment needs. • Completes CBCLs with client and/or guardian once every 6 months from the date of intake and submits to Children’s support staff for scoring. • Reviews CBCL scores with client and/or guardian to monitor client progress in treatment. • Ensures all SED waiver paperwork is completed by client’s family and submitted to DCF in a timely fashion. Paperwork may be facilitated by wraparound facilitator or parent support worker, but is the responsibility of the assigned case manager • Facilitates Psychosocial Rehabilitation-Group and Individual, which are self-contained, goal-directed services designed to assist consumers in minimizing or resolving the effects for mental and emotional impairments. Objectives of the group activities shall be designed to: assist individuals with daily problem solving; improve social skills; provide leisure time training; promote health and enhance personal relationships. • Provides mental health Attendant Care (AC), one-to-one support or supervision with the goal of maintaining the client in natural community locations, such as at home, in school or in the community. Direct support and supervision in activities of daily living or to support the client and/or the family in maintaining daily routines critical to a stable lifestyle.

Job Standards:
• Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. • Possess demonstrated interpersonal skills, ability to work with persons with severe and persistent mental illness and/or serious emotional disturbance, and the ability to react effectively in a wide variety of human service situations. • Meet the specifications as outlined in the CMHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry and motor vehicle screens. • Must complete basic case manager training; intermediate case manager training within first 6 months of employment, may require overnight stay. • Must have a valid Kansas Driver's license and current proof of automobile insurance. • Possess the awareness and judgment to intervene appropriately when dealing with escalating emotional situations. • Possess the ability to negotiate several flights of stairs at a time and move effectively in varied settings throughout the community. • Possess the mobility and judgment to execute personal protective measures. • Able to calmly and effectively deal with agitated individuals or high risk situations. • Demonstrate autonomous critical thinking skills. • Demonstrate good professional judgment and decision-making skills. • Self-motivated. • Able to work with minimal direct supervision. • Demonstrate good time management and organizational skills in a fast-paced environment. • Able to handle competing urgent priorities. • Possess basic or intermediate computer skills: able to learn complex software related to an electronic medical record.

Equipment:
• Possess basic or intermediate computer skills: able to learn complex software related to an electronic medical record. • Must have reliable transportation.

Work Environment:
• Primarily indoors with some work outside engaging with youth in the community.
