Job Description:

Usual working days/hours: Monday - Friday | 8:00 AM - 5:00 PM

Location of work: 2622 W. Central, Suite 500 | Wichita, KS

Job Objectives:
Complete assessments and provide case management services to persons 60 years of age and older residing in Sedgwick, Butler and Harvey Counties requesting Senior Care Act (SCA), In-Home services. This position may also conduct functional eligibility assessments for person 16 years of age and older for the Home and Community Based Services (HCBS) Frail Elderly (FE), Physically Disabled (PD,) and Traumatic Brain Injury (TBI) Waiver programs, or CARE assessments for persons considering nursing home placement.

Essential Job Functions:
• Complete assessment and reassessments for consumers requesting service. o Travel daily to consumers in a tri-county required. o Interview consumers demonstrating active listening skills. o Complete required forms and accurately data enter in required time frame. o Submit accurate and complete documentation and billing. • Provide Case Management to consumers. o Identify individual needs o Develop care plan implementation, and provide on-going monitoring of services and consumer needs. o Educate consumers on community based care and resource options. o Provide accurate and unbiased information or referrals as needed including pertinent materials. • Schedule, complete assignments and documentation with in required deadline. o Complete required documentation and all case notes in required time frame in the electronic client record Management Information System (MIS), Nterfuse. o Maintain an up-to-date Microsoft Outlook calendar. • Provide back up support to other Central Plains Area Agency on Aging (CPAAA) units as needed for assessments. • Special projects and additional duties as needed which supports the Assessment and In-Home Services Unit and agency. • Bilingual staff will interpret as needed.

Job Standards:
Required: Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling, Family Studies, or Human Development. Minimum of one (1) year experience in the geriatric services and/or disabilities field. Minimum of one (1) year of Case Management experience. Valid State of Kansas driver's license and proof of automobile insurance. Preferred: Master’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Two (2) years’ experience in the field of aging or an Area Agency on Aging. Ideal candidate will have experience with physical disabilities (PD), traumatic brain injuries (TBI), persons with a mental health diagnosis, and/or individuals with intellectual or developmental disabilities (ID/DD). Licensed Social Worker or Nurse. Bilingual skills a plus.

Testing/Screening Requested:
• Background checks including but not limited to KBI. • Motor Vehicle Report. • Pre-employment urine drug screen.

Equipment:
Office: Personal Computer, Vehicle, Copier/Fax/Scanner, Multi-line Phone, Printer.

Work Environment:
• Work is conducted 90% in the field. The employee is subject to both environmental/weather conditions. Activities occur inside and outside. • Varied Environmental Conditions: Conditions the employee may encounter include clutter, animals and varied levels of pests.
