Job Objectives:
The Health Links Care Coordinator is focused on improving overall health of persons with severe mental illness and/or chronic physical health conditions. Position is responsible for coordinating the health care needs of participants as defined in the Treatment Plan. Care Coordination is collaboration with the patient, primary care providers, hospitals, specialty providers, mental health providers, and community resources in a team based approach to care that is intended to optimize the overall health of those served. Care coordinators work to improve timely access to care, increase utilization of preventive care, reduce emergency room utilization and hospital readmissions, and reduce health care costs.

Essential Job Functions:
Patient engagement and education • Outreach individuals who have a severe mental illness and those whom have or are at risk of having chronic health conditions such as asthma, COPD, diabetes, and hypertension • Engage members and families to better understand health conditions and motivate them to self-manage and partner in their overall health care • Develop hopeful and trusting relationships • Develop a treatment plan with the patient, family, and providers • Provide individual and group health promotion/education and coaching to members and their supports, including but not limited to the benefits of a healthy lifestyle, good nutrition, physical activity and chronic disease management • Utilize motivational interviewing techniques to facilitate change • Utilize positive reinforcement and encouragement Care Coordination • Be the system navigator and point of contact for members • Address barriers for success, i.e. low income, housing, transportation, health conditions, etc. • Assist members in understanding relevant details from medical appointments including follow through with recommendation and medication compliance • Coordinate access to preventive and health promotion services for chronic conditions as well as mental illness and substance use disorders • Facilitate the transition from inpatient medical or behavioral health care to home and community services systems via coordination with liaisons and treatment team members • Make collateral contact with doctors, family members, designated team members, etc. to assure the needs of the member are being met • Coordinate appointments and assist in accessing services • Demonstrate value of a team approach to care • Identify and foster strong working relationships with other community providers Complete documentation and track progress • Complete documentation in an accurate and timely manner and in compliance with agency, state and federal requirements for coding and content • Ensure confidential communication of patient health information • Monitor and follow up to ensure that needed care is not only offered but accessed • Monitor adherence to treatment plans, evaluate effectiveness, monitor patient progress in a timely manner and facilitate changes as needed • Enter data accurately in electronic health record and program spreadsheets used to track performance measures • Understand and work toward achieving program outcomes as defined by contract(s) and as they relate to overall population health

Job Standards:
• Bachelor’s degree from accredited college or university or be equivalently qualified by work experience or a combination of work experience and training in the field of behavioral or physical health care. • Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. • Meet the specifications as outlined in the CMHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. • Must complete orientations provided by Sedgwick County and COMCARE.

Work Environment:
• Office in a team setting and community settings including client's homes, workplace and other agencies.
