Job Description:

SALARY RANGE:  $18.96/hour at Step 1 to $28.91/hour at Step 20 Depending on Qualifications.

DUTIES & RESPONSIBILITIES: Provide intermittent skilled nursing to patients in their place of residence, in accordance with Agency policy and procedure and State and Federal regulations. Perform thorough head-to-toe assessments on all patients when starting their Home Health care, obtain vital signs with each visit and develop care plans that allow patients to remain in their home.  Communicate with Doctors and obtain orders as required for all skilled care.  Administer medications and treatments as prescribed by Doctor.  Develop care plans to establish goals and necessary nursing interventions. Communicate with Doctors regarding response to all medications and/or reactions / side effects. Obtain laboratory specimens and deliver to Doctors.  Administer IV medications along with saline and heparin flushes. Perform dressing changes to invasive lines. Assist with insulin injections to include education on cleanliness, proper techniques along with educating patients on how to utilize glucometers, cleanliness with finger sticks, appropriate blood sugar ranges and signs & symptoms of hypoglycemia and hyperglycemia.  Provide wound care as ordered by Physician. Change catheters on a regular schedule and as needed. Monitor for signs and symptoms in decline with physical and mental health and provide support and resources to encourage improved mental health. Provide education to patients and family members regarding care of patient in all aspects of medical needs and safety prevention. Schedule and coordinate physical, occupational and speech therapy evaluations. Determine homebound status and patient compliance as required by Medicare.  Electronically document all visits, phone calls, Doctors’ orders (every 60 days) and necessary information. Complete Doctor orders in a timely manner.  Determine payer sources and services provided by insurance providers.  Identify discharge planning needs and implement prior to discharge. Complete all paperwork necessary for Agency policies and Federal/State laws and regulations.  Supervise Home Health Aides every 2 weeks to 60 days. Triage, on-call nights, weekends and holidays. This position is funded through grants, fees and/or general fund. Employment may be subject to continuation of these revenue sources.

Minimum qualifications include: 

  1. Must have a Bachelor’s Degree, Diploma or Associates Degree in nursing.
  2. Must meet licensing requirements by the Kansas State Board of Nursing for Registered Nurses and hold a current licensure for the State of Kansas.
  3. Must have two (2) years experience in nursing preferably the Home Health field.
  4. Must have good communication skills to work with Staff and public in a congenial manner.
  5. An ability to maintain a professional demeanor.
  6. Must have basic knowledge of Excel, Word and other PC software.
  7. A proven ability to plan and organize work.
  8. Must be flexible and be able to adapt to different tasks.
  9. Must have a valid Driver’s License and must provide own transportation.
