Conduct assessments for individuals considering nursing home placement, and functional assessments for persons 16 years of age and older for Home and Community Based Services (HCBS) for Frail Elderly (FE), Physically Disabled (PD), Traumatic Brain Injury (TBI) Waiver and Working Healthy programs.
Essential Job Functions:
• Complete functional assessment for individuals to determine program eligibility for HCBS waiver programs. o Travel daily to consumers in the tri-county area required o Interview Consumer o Complete required forms timely o Submit accurate and complete documentation • Provide Options Counseling to consumers. o Identify individual needs o Effectively and factually communicate and explain to individuals their resource and/or long term care options o Educate consumer regarding long term care options o Provide accurate and unbiased information or referrals as needed including pertinent materials • Complete assignments and documentation within required deadline. o Demonstrate strong interviewing skills and knowledge of assessment procedures.
Minimum qualifications: Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling. Minimum of one (1) year experience in the field of aging. Valid State of Kansas driver s license and proof of automobile insurance.
Preferred qualifications: Master’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Two (2) years’ experience in the field of aging for an Area Agency on Aging. Ideal candidate will have assessment experience with physical disabilities (PD), traumatic brain injuries (TBI), persons with a mental health diagnosis, and/or individuals with intellectual or developmental disabilities (ID/DD). Licensed Social Worker or Nurse. AIRS, Information and Referral Certification. Bilingual skills a plus.
• Background checks including but not limited to KBI. • Motor Vehicle Report. • Pre-employment urine drug screen.
Office: Personal Computer, Copier/Fax/Scanner, Multi-line Phone, Printer.
Work is performed 90% in field in all weather conditions. Will have exposure to wide variety of environmental conditions. Daily travel to consumers is required covering Sedgwick, Butler and Harvey Counties.
