Job Description:

Job Objectives:

Perform Immunization and Health Screening services at all clinic locations.  Provide direct nursing services and education to all citizens at these locations. The CHN I will maintain professional expertise and keep confidentiality in all areas of patient contact. Works under the direction of written medical protocols and standing orders.


Essential Job Functions:

Provide and accurately document direct nursing care including immunizations, health screenings, and education for patients seeking services from the Division of Health at the West Central clinic and off-site at mobile locations. 

  • Administer intramuscular and subcutaneous  immunizations,  perform phlebotomy and capillary blood sampling for lab tests, screen for pediculosis (head lice), place TB skin tests and interpret the results
  • Provide appropriate and effective patient interaction and customer service
  • Provide immunization and health screening services according to DOH protocol
  • Document all services
  • Immunize clients according to ACIP recommendations
  • Participate in clinics outside regular clinic hours

Support Immunization Clinic Services

  • Verify and accurately document vaccine tally count
  • Maintain and track reminder recall system for clients who need further immunizations to include making and documenting postcard mailings and phone calls
  • Prepare treatment room/area for patient care and immunizations
  • Participate in QI project

Vaccine Inventory Management  

  • Accurately document and track vaccines
  • Minimize vaccine wastage by selecting vaccines from the correct funding source for each client
  • Minimize vaccine wastage by selecting and preparing appropriate vaccine

Job Standards:

Minimum Qualifications

Educational Requirements:  Associate degree in Nursing from an accredited school.

License, certifications, etc.    Holds a valid unencumbered nursing license with the Kansas state Board of Nursing.

Preferred Qualifications

Years of Experience:  Experience and/or training in the field of public health.  


Physical Requirements of Position:

Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; reading; visual inspection involving small parts; using measurement devices; and assembly of parts at distances close to the eyes.   The employee is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned.  The employee is required to have visual acuity to operate motor vehicles.

Usual working days/hours:  Mondays - Wednesdays from 8:00 a.m. to 5:30 p.m. / Thursdays from 9:30 a.m. to 7:00 p.m. / Fridays from 8:00 a.m. to 5:00 p.m.
