Job Title: PT Diabetes Educator and Wellness Coordinator
Salary: $18.48 Hourly
Provide direct services to participants in the diabetes education program(s) and fall prevention education services including: diabetes education, fall prevention education, community referrals, one on one education and support primarily for adults 55+. He/she will offer follow-up education for eligible clients and minimal community education for senior groups/senior centers. This position will also consult with staff in Aging to assist with diabetes concerns or address educational needs for both clients and staff.
Essential Job Functions:
- Provide Diabetes education using the AADE curriculum, technical assistance to students and completion of necessary documentation.
- Ensure necessary data (client data, case notes, Medicare billing data) is collected and data entered as appropriate to meet current requirements.
Fall Prevention Educator
- Provide fall prevention education services to community groups.
- Provide A Matter of Balance (AMOB) classes.
Customer Service and Consultation
- Health promotion and disease prevention to community groups and individuals, and increase referrals to the three Diabetes programs.
- Positive communication.
- Work with staff and billing staff to ensure that all necessary paperwork and documentation is accurate and meets Medicare billing standards.
Communication: Clearly conveys and receives information and ideas through the appropriate methods and media for target audiences in a manner that engages the listener, helps them understand and retain the message, and invites response and feedback. Ensures appropriate stakeholders are kept informed and that information shared is accurate and timely. Demonstrates good written, oral, and listening skills.

Educational Requirements: Bachelor’s degree from an accredited college or university in Food, Nutrition or related Science and completes a dietetic route accredited by the Academy of Nutrition and Dietetics.
Years of Experience: Minimum of two (2) years’ of experience as a Dietician or Nurse. Two (2) years’ of experience as a Certified Diabetes Educator (CDE). Experience working with the older adult population.
License, certifications, etc…Licensed in the State of Kansas Registered Dietician or Nurse. Certification: Diabetes Educator (CDE). Valid State of Kansas driver s license and proof of automobile insurance.
Educational Requirements: Bachelor’s Degree in Food, Nutrition or related Science and completes a dietetic route accredited by the Academy of Nutrition and Dietetics.
Years of Experience: Minimum of three (3) years’ experience in the field of nutrition/diabetes education, aging.
License, certifications, etc…Licensed in the State of Kansas: Registered by the Academy of Nutrition and Dietetics.
• Background checks including but not limited to KBI. • Motor Vehicle Report. • Pre-employment urine drug screen.
Usual working days/hours: Monday - Friday, varied
Location of work: 271 W 3rd St N
The employee is typically in an office environment with work conducted 25% in the field. The employee is subject to both environmental/weather conditions. Activities occur inside and outside.
Occasional exposure to environmental/weather conditions when attending meetings, trainings, or other activities.
